Google Form:
- Your students initiate a request with you for proctoring services.
- Instructors complete this Google Form after receiving a student request. Inclusion of the student's Banner number is required.
- Please note any relevant instructions, particularly with regards to allowances related to student accommodations (e.g., if a formula sheet is provided or to be brought by the student). Please be as specific as possible with your instructions.
- Students can take their tests on Canvas/online if you authorize that on the Google Form. Please note, we are unable to monitor their internet usage during their exam. We are only able to monitor what we can see in our cameras.
Next Steps:
- Proctoring staff will email the student a link to schedule an appointment once the form is received. Tentative appointment bookings will be forwarded to the instructor for their reference.
- Instructors will submit all materials for proctoring staff to confirm the appointment. They can be uploaded directly to the Google Form, hand delivered, or emailed to ssctesting@ltu.edu. The student's appointment remains tentative until the exam materials are received.
- Please submit the Google Form and testing materials prior to the day of the exam.
- Any variance from the submitted Google Form must be communicated with the proctoring staff through ssctesting@ltu.edu or through the Google Form. Proctoring staff cannot deviate from the information and instructions provided on the Google Form and testing materials.
- Proctoring staff are not permitted to modify exams. Please submit exactly the exam your student will be taking.
- Students are directed to communicate with you throughout the process. When proctoring staff are communicating with the student, we will make every effort to copy you on the conversation.
Accommodations:
- For students with documented disabilities, refer to the “Accommodations Memo” provided to you by the Access and Accommodations office for the student when determining proctoring instructions.
- Questions about students with disability accommodations should be addressed to the Access and Accommodations office.

During the exam:
- Proctoring staff monitor students taking tests via a camera system and frequent walk throughs.
- If the student is exhibiting any questionable behaviors, or behaviors that do not follow the Academic Honor Code or Student Code of Conduct, proctoring staff will note the time and the instructor will be informed of the situation. Camera footage will be forwarded if necessary.
- Any further action remains at the discretion of the instructor.
Exam returns:
- Exams can be returned to your Dean's Office.
- Exams can be picked up in the Library during proctoring hours.
- Please mark your choice on the Google Form.
